Camden County PARTNERS Differential Response (DR)
Case Management Services

AN OVERVIEW

The Department of Children and Families has funded a Differential Response case management service initiative in Camden County. Differential Response provides coordinated and community based case management services responsive to families experiencing challenges or who have needs that threaten family stability.

TARGET POPULATION and PROFILE

Camden County families referred by the Department of Children and Families Statewide Central Registry (SCR) who have no identified safety, abuse or neglect issues and meet the SCR request for family and child service criteria for child welfare assessment. A referral to Differential Response can be initiated if the family or a concerned community member, i.e. individual and/or agency makes the call.

The SCR referral number is Toll-Free, 24-hours, 7 days a week: 1-877-652-2873. The Differential Response case management initiative is managed by the Community Planning & Advocacy Council (CPAC). Services are provided to the families by the Center for Family Services and The Hispanic Family Center of Southern New Jersey.

PROCESS DESCRIPTION

Differential Response case management provides voluntary, no-cost, coordinated, community-based, family-friendly, culturally responsive, and strength-based services to respond to families that are experiencing challenges or have needs that threaten family stability. 

Differential Response case management uses local services and expertise to provide a comprehensive range of family-centered services to empower families to attain self-sufficiency and promote child safety, permanency and well-being.

 Some of the major features of the Differential Response case management model include:

  • Being telephonically available to receive referrals seven days a week.
  • Having face-to-face contact with the family within 72 Hours of the referral.
  • Engaging the family in all aspects of service delivery and Family Action Plan development.
  • Facilitating an initial Family Team meeting within 10 days of referral.
  • Obtaining the appropriate community-based services and resources.
  • Helping the family to develop and sustain long-term informal supports.
